Problems began when a fire started in a rear lavatory. Prior to the forced landing many of the passengers had already died of carbon monoxide poisoning and smoke inhalation. A possible cause of the fire was that the lavatory waste bin contents caught fire after a lit cigarette was thrown into it. There were 11 survivors.

The carpet beneath Small's body was scorched but there was no other sign of fire in the house. Small's pipe was unlit and on a shelf and his stove lids were all still in place.
